Young people living with HIV face a number of challenges in transitioning from pediatric to adult health care. These challenges are shared among the youth, their health providers and primary caregivers, and can pose a significant threat to young people’s overall health as they manage HIV in adulthood. This workshop will present a project of the Massachusetts Department of Public Health’s Ryan White Part D program, MassCARE, to engage youth living with HIV, their primary caregivers and their health providers in dynamic discussions about health care transition. The resulting recently published guidebook “Moving On Positively”, provides these three populations with practical steps that each can take to prepare for successful transition.
