Steven Bromer, Medical Director of the Practice Facilitation teams is Assistant Professor of Family and Community Medicine, Medical Director of the Sonoma County HIV Healthcare Network, faculty consultant on the National HIV/AIDS Clinicians’ Consultation Center (Warmline, PEPline, Perinatal HIV Hotline) (where he is also Director of Quality Improvement), and is physician for HIV infected and affected families in the Family HIV Clinic at SFGH. He is also co-leader for outpatient electronic medical record redesign at SFGH. Dr. Bromer will be the medical coordinator for the work of the Practice Facilitation teams. He will also be a member of the practice facilitation teams, with particular focus on clinical care in the medical home model at designated clinics. He will review and assess individual work plans, identified problems and overall progress of selected clinics. Because of his electronic medical record experience, he will serve as consultant to all designated Ryan White Clinics needing assistance in this area. As medical director he will review and assess the usefulness of tools used in ensuring medical home status in all designated clinics. He also serves as expert consultant in group consultations and facilitations. Because of his extensive experience in primary care, he serves as telephone consultant on adult primary care issues to Ryan White Clinic clinicians from designated clinics.
