Alison Jordan

Alison O. Jordan, CPPB, LCSW, is the Executive Director for Transitional Health Care Coordination in the Division of Health Care Access and Improvement. In this role, she oversees the program’s activities in coordinating medical discharge planning and providing services and linkages to community health care for those soon to be released from City jails, their visitors, and families. Ms. Jordan oversees the implementation of the discharge planning component of the new HIV Continuum of Care Model for persons leaving City jails. She is also the Principal Investigator for one of 10 sites funded by the HRSA/Special Projects of National Significance grant to conduct a local and multi-site evaluation of linkages for people with HIV/AIDS leaving jails. Prior to joining the Department in 2002, she held progressively responsible positions in the city’s social services agencies, starting as a caseworker and transitioning into human service contracting. She worked at the Mayor’s Office of Contracts and was Deputy Agency Chief Contracting Officer at HRA where she was instrumental in developing and implementing outcome measures and performance indicators for social service contracts. A social worker with extensive knowledge of the City’s community based programs, Ms. Jordan received her MSW degree from the City University of New York’s Hunter College School of Social Work. She currently resides in Bayside, Queens with her family.
