ERIN GAEL CHAMBERS: Prior to working as the HIV ACCESS PCMH Project Director at Alameda Health Consortium, Erin Gael Chambers spent seven years providing medical case management for HIV-positive individuals and overseeing quality management for the Ryan White Program in western Montana. She also has extensive experience in program management from her training and advocacy work at non-profit human services organizations in San Francisco and Seattle. She received her B.A. in Communication Studies from Humboldt State University in 2000 and is currently pursuing her Masters of Public Health through the University of Montana’s School of Public and Community Health Sciences.
